I, the undersigned participant (or parent/legal guardian if the participant is a minor), voluntarily agree to the following terms in consideration of being permitted to participate in Halotherapy (Salt Therapy) and/or Hydrogen Inhalation Services provided by Water & Salt (the “Company”).
Description of Services
Halotherapy (Salt Therapy): Breathing fine dry salt aerosol in a controlled salt room or booth for general wellness and relaxation.
Hydrogen Inhalation: Inhaling a hydrogen-air or hydrogen-oxygen mixture through a machine or nasal cannula for wellness purposes.
These are wellness services only and are not medical treatments.
Acknowledgment of Risks and Side Effects
I understand that these treatments are generally well-tolerated and hardly have side effects for most healthy individuals. However, I still acknowledge the following potential risks and temporary reactions:
Halotherapy risks: Temporary coughing, throat irritation, increased mucus, nasal drip, skin/eye irritation, or temporary worsening of respiratory symptoms.
Hydrogen Inhalation risks: Possible mild dizziness, fatigue, headache, or changes in breathing (rare).
General risks: Allergic reaction, respiratory distress, slipping on salt residue, dehydration, exacerbation of pre-existing conditions, or rare serious medical events. I confirm I have no known contraindications or have consulted my physician. These services are not recommended and may be unsafe if I have/have had: active infections, tuberculosis, fever, severe heart conditions, uncontrolled hypertension, certain cancers, respiratory failure, pregnancy (especially later stages), hyperthyroidism, or any other condition that could be aggravated. I agree to fully disclose my medical history and immediately inform staff of any discomfort during the session. These services are not a substitute for medical care, are not FDA-approved for treating any disease, and are not intended to diagnose, treat, cure, or prevent any illness.
Assumption of Risk and Release of Liability
I voluntarily assume all risks, known and unknown, associated with these services. I hereby release, waive, discharge, and covenant not to sue Water & Salt, its owners, officers, employees, agents, and affiliates (collectively “Releasees”) from any and all liability, claims, demands, or causes of action for any injury, illness, death, or property damage arising from my participation, including those caused by negligence. I agree to indemnify and hold the Releasees harmless from any claims, losses, or expenses (including attorney’s fees) brought by me, my heirs, or others. This release shall be construed under California law and is intended to be as broad as permitted by law. If any part is invalid, the rest remains in effect.
Governing Law & Venue
This Agreement shall be governed by the laws of the State of California. Any disputes shall be resolved exclusively in the courts of Yolo County, California.
Additional Terms
I am at least 18 years old (or my parent/guardian consents).
I am not under the influence of alcohol, drugs, or any impairing substances.
Services are non-refundable.
The Company may refuse service for safety reasons.
I consent to emergency medical treatment if needed, at my own expense.
By signing below, I acknowledge that I have carefully read and fully understand this entire waiver and agree to its terms voluntarily.
